Why the completing party matters more than the content
Read the four columns above in order and a pattern appears. In almost every case the defect that matters is not a wrong value in a box. It is that the wrong person filled the box in, or that the document travelled through a party it should not have touched.
- A verification of employment that reached the employer by way of the borrower has lost the independence that made it evidence.
- A condo questionnaire completed by the listing agent is a sales document wearing a compliance document’s clothing.
- A Direct Endorsement approval without an identifiable underwriter is an approval nobody made.
- A funding fee exemption applied from a borrower’s statement is an exemption VA never granted.
Design the review around provenance first and content second. A file that is complete but whose evidence came from interested parties is worse than a file with a missing page, because it looks finished.

Turning the index into a review
A scope list is not a review. To make it one, each row needs four things recorded separately: whether the document was required for this loan, whether it is present, whether the completing party was acceptable, and what the reperformed test returned. Collapsing those into a single yes or no is how files pass review while carrying the defect the review existed to find.
Keep the source page reference with every result. A reviewer six months later, or an examiner, should be able to open the finding, open the cited page, and reach the same conclusion without asking anyone what happened.

What mortgage teams usually ask.
Why organize a QC reference by form number?
Because that is how the work arrives. A reviewer opens a file and encounters a 1005, a 4506-C and a 92900-A, not an abstract category called employment documentation. Indexing the review by artifact makes it possible to test whether each one was completed by the right party, at the right time, and reconciled to the rest of the file.
Does every closed loan contain all of these?
No. The set varies by product, channel, occupancy, borrower income type and agency. A conventional W-2 purchase and a self-employed FHA condo refinance share very few of these documents. The review scope should be driven by the loan's own attributes.
Are these forms required by the agencies?
Some are and some are not. A 92900-A is required on an FHA file. A Form 1084 is a calculation aid the lender may or may not use. Treat the distinction carefully, because a finding against an optional worksheet is a different conversation from a finding against a required certification.
Where does the authority for any of this sit?
With the current agency guide, handbook or regulation, and with the lender's own approved QC plan. This library is an operating reference for building the evidence a decision needs. It is not a substitute for either.
